Ordering Form For Mielke's Fiber Arts, LLC

NOTICE: In preparation for our trip to Michigan Fiber Festival, our last shipping day before the festival will be Monday, August 4th, 2008. We will accept phone orders up until we leave, and web orders even while we are gone (but we won't be able to respond to web orders until we return). We will resume shipping once we return and unpack - which we estimate to be Tuesday, August 19th, 2008.

Fill this form out completely and submit. We will email (or in rare instances, call) you with a total that includes shipping, at which time you can choose to pay by credit card or PayPal. Please allow up to 24 hours for our response and note that we are slower on the weekends, when it is busier here. We physically type up your invoice - it's not computer generated!

Orders shipped to Wisconsin will include WI and local sales tax. Orders shipped to Illinois will include a 6.25% sales tax.

Order payment can be made by PayPal, Visa or MasterCard. PLEASE do not send your credit card information in an email; for PayPal and credit card orders, we will email you a total for your order along with a link to a secure page to leave your card info on.
